Are you looking to help someone else?
To help someone else sort out benefit problems or make a claim you need to get their ‘informed consent’ first.
‘Informed consent’ means they have;
- agreed you can make calls, write letters and make decisions for them AND
- signed a consent form to show they have agreed to this OR
- they are always there with you when you speak to an adviser or benefit office about their claim
Use the template Consent Form below if the person you want to help agrees to give informed consent.

You don’t need their consent just to look for information. Choose Advice and Information below to do that.
